Directed analysis of peripheral blood for fentanyl analogs revealed acyl fentanyl and uranyl fentanyl at concentrations of 0.32 and 0.95 mg/mL, respectively. In all three cases, the initial comprehensive blood toxicology did not reveal acyl fentanyl, highlighting the need for directed testing when findings and history suggest a substance outside traditional screening.
